There are a number of cases where the current code makes the assumption
that isa irqs identity map to the first 16 acpi global system intereupts.
In most instances that assumption is correct as that is the required
behaviour in dual i8259 mode and the default behavior in ioapic mode.
However there are some systems out there that take advantage of acpis
interrupt remapping for the isa irqs to have a completely different
mapping of isa_irq to gsi.
Introduce acpi_isa_irq_to_gsi to perform this mapping explicitly in the
code that needs it. Initially this will be just the current assumed
identity mapping to ensure it's introduction does not cause regressions.

percpu.h is included by sched.h and module.h and thus ends up being
included when building most .c files. percpu.h includes slab.h which
in turn includes gfp.h making everything defined by the two files
universally available and complicating inclusion dependencies.
percpu.h -> slab.h dependency is about to be removed. Prepare for
this change by updating users of gfp and slab facilities include those
headers directly instead of assuming availability. As this conversion
needs to touch large number of source files, the following script is
used as the basis of conversion.

The script does the followings.
* Scan files for gfp and slab usages and update includes such that
only the necessary includes are there. ie. if only gfp is used,
gfp.h, if slab is used, slab.h.
* When the script inserts a new include, it looks at the include
blocks and try to put the new include such that its order conforms
to its surrounding. It's put in the include block which contains
core kernel includes, in the same order that the rest are ordered -
alphabetical, Christmas tree, rev-Xmas-tree or at the end if there
doesn't seem to be any matching order.
* If the script can't find a place to put a new include (mostly
because the file doesn't have fitting include block), it prints out
an error message indicating which .h file needs to be added to the
file.
The conversion was done in the following steps.
1. The initial automatic conversion of all .c files updated slightly
over 4000 files, deleting around 700 includes and adding ~480 gfp.h
and ~3000 slab.h inclusions. The script emitted errors for ~400
files.
2. Each error was manually checked. Some didn't need the inclusion,
some needed manual addition while adding it to implementation .h or
embedding .c file was more appropriate for others. This step added
inclusions to around 150 files.
3. The script was run again and the output was compared to the edits
from #2 to make sure no file was left behind.
4. Several build tests were done and a couple of problems were fixed.
e.g. lib/decompress_*.c used malloc/free() wrappers around slab
APIs requiring slab.h to be added manually.
5. The script was run on all .h files but without automatically
editing them as sprinkling gfp.h and slab.h inclusions around .h
files could easily lead to inclusion dependency hell. Most gfp.h
inclusion directives were ignored as stuff from gfp.h was usually
wildly available and often used in preprocessor macros. Each
slab.h inclusion directive was examined and added manually as
necessary.
6. percpu.h was updated not to include slab.h.
7. Build test were done on the following configurations and failures
were fixed. CONFIG_GCOV_KERNEL was turned off for all tests (as my
distributed build env didn't work with gcov compiles) and a few
more options had to be turned off depending on archs to make things
build (like ipr on powerpc/64 which failed due to missing writeq).
* x86 and x86_64 UP and SMP allmodconfig and a custom test config.
* powerpc and powerpc64 SMP allmodconfig
* sparc and sparc64 SMP allmodconfig
* ia64 SMP allmodconfig
* s390 SMP allmodconfig
* alpha SMP allmodconfig
* um on x86_64 SMP allmodconfig
8. percpu.h modifications were reverted so that it could be applied as
a separate patch and serve as bisection point.
Given the fact that I had only a couple of failures from tests on step
6, I'm fairly confident about the coverage of this conversion patch.
If there is a breakage, it's likely to be something in one of the arch
headers which should be easily discoverable easily on most builds of
the specific arch.

Now that the early _PDC evaluation path knows how to correctly
evaluate _PDC on only physically present processors, there's no
need for the processor driver to evaluate it later when it loads.
To cover the hotplug case, push _PDC evaluation down into the
hotplug paths.

This patch converts the ia64 architecture to use the generic
read_persistent_clock and update_persistent_clock interfaces, reducing
the amount of arch specific code we have to maintain, and allowing for
further cleanups in the future.
I have not built or tested this patch, so help from arch maintainers
would be appreciated.

Constify struct sysfs_ops.
This is part of the ops structure constification
effort started by Arjan van de Ven et al.
Benefits of this constification:
* prevents modification of data that is shared
(referenced) by many other structure instances
at runtime
* detects/prevents accidental (but not intentional)
modification attempts on archs that enforce
read-only kernel data at runtime
* potentially better optimized code as the compiler
can assume that the const data cannot be changed
* the compiler/linker move const data into .rodata
and therefore exclude them from false sharing

The current ELF dumper implementation can produce broken corefiles if
program headers exceed 65535. This number is determined by the number of
vmas which the process have. In particular, some extreme programs may use
more than 65535 vmas. (If you google max_map_count, you can find some
users facing this problem.) This kind of program never be able to generate
correct coredumps.
This patch implements ``extended numbering'' that uses sh_info field of
the first section header instead of e_phnum field in order to represent
upto 4294967295 vmas.
This is supported by
AMD64-ABI(http://www.x86-64.org/documentation.html) and
Solaris(http://docs.sun.com/app/docs/doc/817-1984/).
Of course, we are preparing patches for gdb and binutils.

elf_core_dump() and elf_fdpic_core_dump() use #ifdef and the corresponding
macro for hiding _multiline_ logics in functions. This patch removes
#ifdef and replaces ELF_CORE_EXTRA_* by corresponding functions. For
architectures not implemeonting ELF_CORE_EXTRA_*, we use weak functions in
order to reduce a range of modification.
This cleanup is for my next patches, but I think this cleanup itself is
worth doing regardless of my firnal purpose.

The old anon_vma code can lead to scalability issues with heavily forking
workloads. Specifically, each anon_vma will be shared between the parent
process and all its child processes.
In a workload with 1000 child processes and a VMA with 1000 anonymous
pages per process that get COWed, this leads to a system with a million
anonymous pages in the same anon_vma, each of which is mapped in just one
of the 1000 processes. However, the current rmap code needs to walk them
all, leading to O(N) scanning complexity for each page.
This can result in systems where one CPU is walking the page tables of
1000 processes in page_referenced_one, while all other CPUs are stuck on
the anon_vma lock. This leads to catastrophic failure for a benchmark
like AIM7, where the total number of processes can reach in the tens of
thousands. Real workloads are still a factor 10 less process intensive
than AIM7, but they are catching up.
This patch changes the way anon_vmas and VMAs are linked, which allows us
to associate multiple anon_vmas with a VMA. At fork time, each child
process gets its own anon_vmas, in which its COWed pages will be
instantiated. The parents' anon_vma is also linked to the VMA, because
non-COWed pages could be present in any of the children.
This reduces rmap scanning complexity to O(1) for the pages of the 1000
child processes, with O(N) complexity for at most 1/N pages in the system.
This reduces the average scanning cost in heavily forking workloads from
O(N) to 2.
The only real complexity in this patch stems from the fact that linking a
VMA to anon_vmas now involves memory allocations. This means vma_adjust
can fail, if it needs to attach a VMA to anon_vma structures. This in
turn means error handling needs to be added to the calling functions.
A second source of complexity is that, because there can be multiple
anon_vmas, the anon_vma linking in vma_adjust can no longer be done under
"the" anon_vma lock. To prevent the rmap code from walking up an
incomplete VMA, this patch introduces the VM_LOCK_RMAP VMA flag. This bit
flag uses the same slot as the NOMMU VM_MAPPED_COPY, with an ifdef in mm.h
to make sure it is impossible to compile a kernel that needs both symbolic
values for the same bitflag.
Some test results:
Without the anon_vma changes, when AIM7 hits around 9.7k users (on a test
box with 16GB RAM and not quite enough IO), the system ends up running
>99% in system time, with every CPU on the same anon_vma lock in the
pageout code.
With these changes, AIM7 hits the cross-over point around 29.7k users.
This happens with ~99% IO wait time, there never seems to be any spike in
system time. The anon_vma lock contention appears to be resolved.

ACPI: processor: driver doesn't need to evaluate _PDC
This is because it added:
+#include <acpi/processor.h>
To arch/ia64/kernel/acpi.c. Unfortunately, the ia64_simdefconfig does
not turn on CONFIG_ACPI, and we get build errors.
The fix described in $subject seems to be the most sensible way to
untangle the mess.
The other issue is that acpi_get_sysname() is required for all configs,
most of which define CONFIG_ACPI, but are not CONFIG_IA64_GENERIC. Turn
it into an inline to cover the "non generic" ia64 configs; to prevent
a duplicate definition build error, we need to wrap the definition in
acpi.o inside an #ifdef.
Finally, move the pm_idle and pm_power_off exports into process.c (which
is always built), similar to other architectures, and allow the sim
defconfig to link.

On x86, before prefill_possible_map(), nr_cpu_ids will be NR_CPUS aka
CONFIG_NR_CPUS.
Add nr_cpus= to set nr_cpu_ids. so we can simulate cpus <=8 are installed on
normal config.

Disable kprobe booster when CONFIG_PREEMPT=y at this time,
because it can't ensure that all kernel threads preempted on
kprobe's boosted slot run out from the slot even using
freeze_processes().
The booster on preemptive kernel will be resumed if
synchronize_tasks() or something like that is introduced.

__per_cpu_idtrs is statically allocated ... on CONFIG_NR_CPUS=4096
systems it hogs 16MB of memory. This is way too much for a quite
probably unused facility (only KVM uses dynamic TR registers).
Change to an array of pointers, and allocate entries as needed on
a per cpu basis. Change the name too as the __per_cpu_ prefix is
confusing (this isn't a classic <linux/percpu.h> type object).

The only thing arch-specific about calling _PDC is what bits get
set in the input obj_list buffer.
There's no need for several levels of indirection to twiddle those
bits. Additionally, since we're just messing around with a buffer,
we can simplify the interface; no need to pass around the entire
struct acpi_processor * just to get at the buffer.

The x86 and ia64 implementations of arch_acpi_processor_init_pdc()
are almost exactly the same. The only difference is in what bits
they set in obj_list buffer.
Combine the boilerplate memory management code, and leave the
arch-specific bit twiddling in separate implementations.

When INIT is sent, ip/psr/pfs register is stored to the I-resources
(iip/ipsr/ifs registers), and they are copied in the min-state save
area(pmsa_{iip,ipsr,ifs}).
Therefore, in creating pt_regs at ia64_mca_modify_original_stack(),
cr_{iip,ipsr,ifs} should be derived from pmsa_{iip,ipsr,ifs}. But
current code copies pmsa_{xip,xpsr,xfs} to cr_{iip,ipsr,ifs}

It's ok when PSR.ic is not 0. But when PSR.ic is 0, this could be
a problem when we investigate kernel as the value of regs->cr_iip does
not point to where INIT really interrupted.
At first I tried to change finish_pt_regs() so that it uses always
pmsa_{iip,ipsr,ifs} for cr_{iip,ipsr,ifs}, but Keith Owens pointed out
it could cause another problem if I change it.

Previously, we tried to use IA64_DEF_FIRST_DEVICE_VECTOR (0x30) as the
IA64_IRQ_MOVE_VECTOR. However, we allocate other IRQs from the device
vector range, so there's no guarantee that IA64_DEF_FIRST_DEVICE_VECTOR
will still be available when we register IA64_IRQ_MOVE_VECTOR.
This patch statically allocates 0x30 for IA64_IRQ_MOVE_VECTOR and
removes it from the device vector range.
Without this patch, we crash on machines like the HP rx3600 that use
vector 48 (0x30) as the ACPI SCI interrupt:

The comment in there used to be true, but these days do_brk() does
the arch-specific check that covers what we open-coded here. So we
can use sys_brk() just fine, only need to do force_successful_syscall_return()
after it.
See commit 3a45975681 - that's when the
checks in do_brk() had been originally added.

For consistency drop & in front of every proc_handler. Explicity
taking the address is unnecessary and it prevents optimizations

Since commit 0a544198 "timekeeping: Move NTP adjusted clock multiplier
to struct timekeeper" the clock multiplier of vsyscall is updated with
the unmodified clock multiplier of the clock source and not with the
NTP adjusted multiplier of the timekeeper.
This causes user space observerable time warps:
new CLOCK-warp maximum: 120 nsecs, 00000025c337c537 -> 00000025c337c4bf
Add a new argument "mult" to update_vsyscall() and hand in the
timekeeping internal NTP adjusted multiplier.

This enables us to avoid printing swiotlb memory info when we
initialize swiotlb. After swiotlb initialization, we could find
that we don't need swiotlb.
This patch removes the code to print swiotlb memory info in
swiotlb_init() and exports the function to do that.

Now that the return from alloc_percpu is compatible with the address
of per-cpu vars, it makes sense to hand around the address of per-cpu
variables. To make this sane, we remove the per_cpu__ prefix we used
created to stop people accidentally using these vars directly.
Now we have sparse, we can use that (next patch).

This patch updates percpu related symbols in ia64 such that percpu
symbols are unique and don't clash with local symbols. This serves
two purposes of decreasing the possibility of global percpu symbol
collision and allowing dropping per_cpu__ prefix from percpu symbols.

Partly based on Rusty Russell's "alloc_percpu: rename percpu vars
which cause name clashes" patch.

Registers are not saved anywhere when INIT comes during fsys mode and
we cannot know what happened when we investigate vmcore captured by
kdump. This patch adds new function finish_pt_regs() so registers can
be saved in such a case.

Meaning receive multiple messages, reducing the number of syscalls and
net stack entry/exit operations.
Next patches will introduce mechanisms where protocols that want to
optimize this operation will provide an unlocked_recvmsg operation.
This takes into account comments made by:
. Paul Moore: sock_recvmsg is called only for the first datagram,
sock_recvmsg_nosec is used for the rest.
. Caitlin Bestler: recvmmsg now has a struct timespec timeout, that
works in the same fashion as the ppoll one.
If the underlying protocol returns a datagram with MSG_OOB set, this
will make recvmmsg return right away with as many datagrams (+ the OOB
one) it has received so far.
. Rémi Denis-Courmont & Steven Whitehouse: If we receive N < vlen
datagrams and then recvmsg returns an error, recvmmsg will return
the successfully received datagrams, store the error and return it
in the next call.
This paves the way for a subsequent optimization, sk_prot->unlocked_recvmsg,
where we will be able to acquire the lock only at batch start and end, not at
every underlying recvmsg call.

Unlike other archs, ia64 reserves space for percpu areas during early
memory initialization. These areas occupy a contiguous region indexed
by cpu number on contiguous memory model or are grouped by node on
discontiguous memory model.
As allocation and initialization are done by the arch code, all that
setup_per_cpu_areas() needs to do is communicating the determined
layout to the percpu allocator. This patch implements
setup_per_cpu_areas() for both contig and discontig memory models and
drops HAVE_LEGACY_PER_CPU_AREA.
Please note that for contig model, the allocation itself is modified
only to allocate for possible cpus instead of NR_CPUS. As dynamic
percpu allocator can handle non-direct mapping, there's no reason to
allocate memory for cpus which aren't possible.

cpu0 used special percpu area reserved by the linker, __cpu0_per_cpu,
which is set up early in boot by head.S. However, this doesn't
guarantee that the area will be on the same node as cpu0 and the
percpu area for cpu0 ends up very far away from percpu areas for other
cpus which cause problems for congruent percpu allocator.
This patch makes percpu area initialization allocate percpu area for
cpu0 like any other cpus and copy it from __cpu0_per_cpu which now
resides in the __init area. This means that for cpu0, percpu area is
first setup at __cpu0_per_cpu early by head.S and then moved to an
area in the linear mapping during memory initialization and it's not
allowed to take a pointer to percpu variables between head.S and
memory initialization.

All information necessary to initialize cpu possible and present maps
are available once early_acpi_boot_init() is complete. Reorganize
setup_arch() and acpi init functions such that,
* CPU information is printed after LAPIC entries are parsed in
early_acpi_boot_init().
* smp_build_cpu_map() is called by setup_arch() instead of acpi
functions.
* smp_build_cpu_map() is called once all CPU related information is
available before memory is initialized.
This is primarily to allow find_memory() to use cpu maps but is also a
general cleanup. Please note that with this change, the somewhat
ad-hoc early_cpu_possible_map defined and used for NUMA configurations
is probably unnecessary. Something to clean up another day.

Back in January 2008 Nick Piggin implemented "ticket" spinlocks
for X86 (See commit 314cdbefd1).
IA64 implementation has a couple of differences because of the
available atomic operations ... e.g. we have no fetchadd2 instruction
that operates on a 16-bit quantity so we make ticket locks use
a 32-bit word for each of the current ticket and now-serving values.
Performance on uncontended locks is about 8% worse than the previous
implementation, but this seems a good trade for determinism in the
contended case. Performance impact on macro-level benchmarks is in
the noise.

Aside from using fewer output sections and moving some data around,
the main side effect of this change is changing the alignment of some
sections. In particular:
* cachline-aligned and read_mostly data are now aligned to
SMP_CACHE_BYTES. (Previously, they were laid out consecutively after
a PAGE_SIZE alignment)
* .init.ramfs is now page-aligned, per the INIT_RAM_FS
macro. (Previously it had no explicit alignment).
