Impact: remove 32-bit optimization to prepare unification
x86-32 and -64 differ in the way they context-switch tasks
with io permission bitmaps. x86-64 simply copies the next
tasks io bitmap into place (if any) on context switch. x86-32
invalidates the bitmap on context switch, so that the next
IO instruction will fault; at that point it installs the
appropriate IO bitmap.
This makes context switching IO-bitmap-using tasks a bit more
less expensive, at the cost of making the next IO instruction
slower due to the extra fault. This tradeoff only makes sense
if IO-bitmap-using processes are relatively common, but they
don't actually use IO instructions very often.
However, in a typical desktop system, the only process likely
to be using IO bitmaps is the X server, and nothing at all on
a server. Therefore the lazy context switch doesn't really win
all that much, and its just a gratuitious difference from
64-bit code.
This patch removes the lazy context switch, with a view to
unifying this code in a later change.

Some syscalls need to access the pt_regs structure, either to copy
user register state or to modifiy it. This patch adds stubs to load
the address of the pt_regs struct into the %eax register, and changes
the syscalls to take the pointer as an argument instead of relying on
the assumption that the pt_regs structure overlaps the function
arguments.
Drop the use of regparm(1) due to concern about gcc bugs, and to move
in the direction of the eventual removal of regparm(0) for asmlinkage.

We have a lot of code which differs only by the naming of specific
members of structures that contain registers. In order to enable
additional unifications, this patch drops the e- or r- size prefix
from the register names in struct pt_regs, and drops the x- prefixes
for segment registers on the 32-bit side.
This patch also performs the equivalent renames in some additional
places that might be candidates for unification in the future.

Simplify set_bitmap(). This is not in a hotpath and we really can use the
straight forward loop through those bits. A similar implementation is used
in the 64 bit code as well.

Since the x86 merge, lots of files that referenced their own filenames
are no longer correct. Rather than keep them up to date, just delete
them, as they add no real value.
Additionally:
- fix up comment formatting in scx200_32.c
- Remove a credit from myself in setup_64.c from a time when we had no SCM
- remove longwinded history from tsc_32.c which can be figured out from
git.

lguest defines its own TSS struct because the "struct tss_struct"
contains linux-specific additions. Andi asked me to split the struct
in processor.h.
Unfortunately it makes usage a little awkward.

Use thread info flags to track use of debug registers and IO bitmaps.
- add TIF_DEBUG to track when debug registers are active
- add TIF_IO_BITMAP to track when I/O bitmap is used
- modify __switch_to() to use the new TIF flags

my patch "x86: initialise tss->io_bitmap_owner to something" (commit ID
d5cd4aadd3) introduced a problem with a
program (DOSEMU) that called ioperm after already doing some port i/o.
The problem is that a process switch return causes tss->io_bitmap_base
to be set to IO_BITMAP_OFFSET so that the fault (that *really* sets the
io bitmap) never triggers.
This fixes that regression.

The pushf/popf in switch_to are ONLY used to switch IOPL. Making this
explicit in C code is more clear. This pushf/popf pair was added as a
bugfix for leaking IOPL to unprivileged processes when using
sysenter/sysexit based system calls (sysexit does not restore flags).
When requesting an IOPL change in sys_iopl(), it is just as easy to change
the current flags and the flags in the stack image (in case an IRET is
required), but there is no reason to force an IRET if we came in from the
SYSENTER path.
This change is the minimal solution for supporting a paravirtualized Linux
kernel that allows user processes to run with I/O privilege. Other
solutions require radical rewrites of part of the low level fault / system
call handling code, or do not fully support sysenter based system calls.
Unfortunately, this added one field to the thread_struct. But as a bonus,
on P4, the fastest time measured for switch_to() went from 312 to 260
cycles, a win of about 17% in the fast case through this performance
critical path.

Initial git repository build. I'm not bothering with the full history,
even though we have it. We can create a separate "historical" git
archive of that later if we want to, and in the meantime it's about
3.2GB when imported into git - space that would just make the early
git days unnecessarily complicated, when we don't have a lot of good
infrastructure for it.
Let it rip!
