In pseries/lpar.c, fix some printf specifier mismatches, and add
a newline to one printk.
In pseries/rtasd.c add "rtasd" to some messages to make it clear
where they're coming from.
In pseries/scanlog.c remove the hand-rolled runtime debugging support
in there. This file has been largely unchanged for eons, if we need to
debug it in future we can recompile.

scanlog_init() could use some love.
* properly return -ENODEV if this system doesn't support scan-log-dump
* don't printk if scan-log-dump not present; only older systems have it
* convert from create_proc_entry() to preferred proc_create()

Many struct file_operations in the kernel can be "const". Marking them const
moves these to the .rodata section, which avoids false sharing with potential
dirty data. In addition it'll catch accidental writes at compile time to
these shared resources.

scanlog.c is only compiled on pSeries. Thus, this patch moves it to
platforms/pseries.
Built and booted on pSeries LPAR (ARCH=powerpc and ARCH=ppc64). Built
for iSeries (ARCH=powerpc).

Use msleep_interruptible() instead of schedule_timeout() in ppc64-specific
code to cleanup/simplify the sleeping logic. Change the units of the
parameter of do_event_scan_all_cpus() to milliseconds from jiffies. The
return value of rtas_extended_busy_delay_time() was incorrectly being used
as a jiffies value (it is actually milliseconds), which is fixed by using
the value as a parameter to msleep_interruptible(). Also, use
rtas_extended_busy_delay_time() in another case where similar logic is
duplicated.

Initial git repository build. I'm not bothering with the full history,
even though we have it. We can create a separate "historical" git
archive of that later if we want to, and in the meantime it's about
3.2GB when imported into git - space that would just make the early
git days unnecessarily complicated, when we don't have a lot of good
infrastructure for it.
Let it rip!
