Change header guards named "ASM_X86__*" to "_ASM_X86_*" since:
a. the double underscore is ugly and pointless.
b. no leading underscore violates namespace constraints.

This patch is the result of an automatic script that consolidates the
format of all the headers in include/asm-x86/.
The format:
1. No leading underscore. Names with leading underscores are reserved.
2. Pathname components are separated by two underscores. So we can
distinguish between mm_types.h and mm/types.h.
3. Everything except letters and numbers are turned into single
underscores.

reboot_{32|64}.c unification patch.
This patch unifies the code from the reboot_32.c and reboot_64.c files.
It has been tested in computers with X86_32 and X86_64 kernels and it
looks like all reboot modes work fine (EFI restart system hasn't been
tested yet).
Probably I made some mistakes (like I usually do) so I hope
we can identify and fix them soon.

set_cpus_allowed is not safe in interrupt context
and disabling apics is complicated code so don't
call machine_shutdown on i386 from emergency_restart().

When the kernel is working well and we want to restart cleanly
kernel_restart is the function to use. But in many instances
the kernel wants to reboot when thing are expected to be working
very badly such as from panic or a software watchdog handler.
This patch adds the function emergency_restart() so that
callers can be clear what semantics they expect when calling
restart. emergency_restart() is expected to be callable
from interrupt context and possibly reliable in even more
trying circumstances.
This is an initial generic implementation for all architectures.
