On VIVT ARM, when we have multiple shared mappings of the same file
in the same MM, we need to ensure that we have coherency across all
copies. We do this via make_coherent() by making the pages
uncacheable.
This used to work fine, until we allowed highmem with highpte - we
now have a page table which is mapped as required, and is not available
for modification via update_mmu_cache().
Ralf Beache suggested getting rid of the PTE value passed to
update_mmu_cache():
On MIPS update_mmu_cache() calls __update_tlb() which walks pagetables
to construct a pointer to the pte again. Passing a pte_t * is much
more elegant. Maybe we might even replace the pte argument with the
pte_t?
Ben Herrenschmidt would also like the pte pointer for PowerPC:
Passing the ptep in there is exactly what I want. I want that
-instead- of the PTE value, because I have issue on some ppc cases,
for I$/D$ coherency, where set_pte_at() may decide to mask out the
_PAGE_EXEC.
So, pass in the mapped page table pointer into update_mmu_cache(), and
remove the PTE value, updating all implementations and call sites to
suit.

If CONFIG_VIRTUAL_MEM_MAP is enabled, ia64 defines macro VMALLOC_END
as unsigned long variable vmalloc_end which is adjusted to prepare
room for vmemmap. This becomes probnlematic if a local variables
vmalloc_end is defined in some function (not very unlikely) and
VMALLOC_END is used in the function - the function thinks its
referencing the global VMALLOC_END value but would be referencing its
own local vmalloc_end variable.
There's no reason VMALLOC_END should be a macro. Just define it as an
unsigned long variable if CONFIG_VIRTUAL_MEM_MAP is set to avoid nasty
surprises.

After moving the the include files there were a few clean-ups:
1) Some files used #include <asm-ia64/xyz.h>, changed to <asm/xyz.h>
2) Some comments alerted maintainers to look at various header files to
make matching updates if certain code were to be changed. Updated these
comments to use the new include paths.
3) Some header files mentioned their own names in initial comments. Just
deleted these self references.

s390 for one, cannot implement VM_MIXEDMAP with pfn_valid, due to their memory
model (which is more dynamic than most). Instead, they had proposed to
implement it with an additional path through vm_normal_page(), using a bit in
the pte to determine whether or not the page should be refcounted:

This is fine, however if we are allowed to use a bit in the pte to determine
refcountedness, we can use that to _completely_ replace all the vma based
schemes. So instead of adding more cases to the already complex vma-based
scheme, we can have a clearly seperate and simple pte-based scheme (and get
slightly better code generation in the process):

And finally, we may rather make this concept usable by any architecture rather
than making it s390 only, so implement a new type of pte state for this.
Unfortunately the old vma based code must stay, because some architectures may
not be able to spare pte bits. This makes vm_normal_page a little bit more
ugly than we would like, but the 2 cases are clearly seperate.
So introduce a pte_special pte state, and use it in mm/memory.c. It is
currently a noop for all architectures, so this doesn't actually result in any
compiled code changes to mm/memory.o.
BTW:
I haven't put vm_normal_page() into arch code as-per an earlier suggestion.
The reason is that, regardless of where vm_normal_page is actually
implemented, the *abstraction* is still exactly the same. Also, while it
depends on whether the architecture has pte_special or not, that is the
only two possible cases, and it really isn't an arch specific function --
the role of the arch code should be to provide primitive functions and
accessors with which to build the core code; pte_special does that. We do
not want architectures to know or care about vm_normal_page itself, and
we definitely don't want them being able to invent something new there
out of sight of mm/ code. If we made vm_normal_page an arch function, then
we have to make vm_insert_mixed (next patch) an arch function too. So I
don't think moving it to arch code fundamentally improves any abstractions,
while it does practically make the code more difficult to follow, for both
mm and arch developers, and easier to misuse.

Current ia64 kernel flushes icache by lazy_mmu_prot_update() *after*
set_pte(). This is too late. This patch removes lazy_mmu_prot_update and
add modfied set_pte() for flushing if necessary.
This patch flush icache of a page when
new pte has exec bit.
&& new pte has present bit
&& new pte is user's page.
&& (old *ptep is not present
|| new pte's pfn is not same to old *ptep's ptn)
&& new pte's page has no Pg_arch_1 bit.
Pg_arch_1 is set when a page is cache consistent.
I think this condition checks are much easier to understand than considering
"Where sync_icache_dcache() should be inserted ?".
pte_user() for ia64 was removed by http://lkml.org/lkml/2007/6/12/67 as
clean-up. So, I added it again.

Equip IA64 sparsemem with a virtual memmap. This is similar to the existing
CONFIG_VIRTUAL_MEM_MAP functionality for DISCONTIGMEM. It uses a PAGE_SIZE
mapping.
This is provided as a minimally intrusive solution. We split the 128TB
VMALLOC area into two 64TB areas and use one for the virtual memmap.
This should replace CONFIG_VIRTUAL_MEM_MAP long term.

Some changes done a while ago to avoid pounding on ptep_set_access_flags and
update_mmu_cache in some race situations break sun4c which requires
update_mmu_cache() to always be called on minor faults.
This patch reworks ptep_set_access_flags() semantics, implementations and
callers so that it's now responsible for returning whether an update is
necessary or not (basically whether the PTE actually changed). This allow
fixing the sparc implementation to always return 1 on sun4c.

Most architectures defined three macros, MK_IOSPACE_PFN(), GET_IOSPACE()
and GET_PFN() in pgtable.h. However, the only callers of any of these
macros are in Sparc specific code, either in arch/sparc, arch/sparc64 or
drivers/sbus.
This patch removes the redundant macros from all architectures except
sparc and sparc64.

One of the changes necessary for shared page tables is to standardize the
pxx_page macros. pte_page and pmd_page have always returned the struct
page associated with their entry, while pte_page_kernel and pmd_page_kernel
have returned the kernel virtual address. pud_page and pgd_page, on the
other hand, return the kernel virtual address.
Shared page tables needs pud_page and pgd_page to return the actual page
structures. There are very few actual users of these functions, so it is
simple to standardize their usage.
Since this is basic cleanup, I am submitting these changes as a standalone
patch. Per Hugh Dickins' comments about it, I am also changing the
pxx_page_kernel macros to pxx_page_vaddr to clarify their meaning.

This closes a couple holes in our attribute aliasing avoidance scheme:
- The current kernel fails mmaps of some /dev/mem MMIO regions because
they don't appear in the EFI memory map. This keeps X from working
on the Intel Tiger box.
- The current kernel allows UC mmap of the 0-1MB region of
/sys/.../legacy_mem even when the chipset doesn't support UC
access. This causes an MCA when starting X on HP rx7620 and rx8620
boxes in the default configuration.
There's more detail in the Documentation/ia64/aliasing.txt file this
adds, but the general idea is that if a region might be covered by
a granule-sized kernel identity mapping, any access via /dev/mem or
mmap must use the same attribute as the identity mapping.
Otherwise, we fall back to using an attribute that is supported
according to the EFI memory map, or to using UC if the EFI memory
map doesn't mention the region.

The optional hugepage callback, hugetlb_free_pgd_range() is presently
implemented non-trivially only on ia64 (but I plan to add one for powerpc
shortly). It has its own prototype for the function in asm-ia64/pgtable.h.
However, since the function is called from generic code, it make sense for
its prototype to be in the generic hugetlb.h header file, as the protypes
other arch callbacks already are (prepare_hugepage_range(),
set_huge_pte_at(), etc.). This patch makes it so.

2.6.16-rc3 uses hugetlb on-demand paging, but it doesn_t support hugetlb
mprotect.

Remove a test from the mprotect() path which checks that the mprotect()ed
range on a hugepage VMA is hugepage aligned (yes, really, the sense of
is_aligned_hugepage_range() is the opposite of what you'd guess :-/).
In fact, we don't need this test. If the given addresses match the
beginning/end of a hugepage VMA they must already be suitably aligned. If
they don't, then mprotect_fixup() will attempt to split the VMA. The very
first test in split_vma() will check for a badly aligned address on a
hugepage VMA and return -EINVAL if necessary.

On i386 and x86-64, pte flag _PAGE_PSE collides with _PAGE_PROTNONE. The
identify of hugetlb pte is lost when changing page protection via mprotect.
A page fault occurs later will trigger a bug check in huge_pte_alloc().
The fix is to always make new pte a hugetlb pte and also to clean up
legacy code where _PAGE_PRESENT is forced on in the pre-faulting day.

This patch introduces 4-level page tables to ia64. I have run
some benchmarks and found nothing interesting. Performance has
consistently fallen within the noise range.
It also introduces a config option (setting the default to 3
levels). The config option prevents having 4 level page
tables with 64k base page size.

This patch removes page_pte_prot and page_pte macros from all
architectures. Some architectures define both, some only page_pte (broken)
and others none. These macros are not used anywhere.
page_pte_prot(page, prot) is identical to mk_pte(page, prot) and
page_pte(page) is identical to page_pte_prot(page, __pgprot(0)).

Currently, region numbers are defined in several files, with several
names. For example, we have REGION_KERNEL in asm/page.h and
RGN_KERNEL in pgtable.h
We also have address definitions that should depend on the
RGN_XXX macros, but are currently just long constants.
The following patch reorganises all the definitions so that they have
the same form (RGN_XXX), are in one place, and that addresses that
depend on RGN_XXX are derived from them.
(This is a necessary but not sufficient patch to allow UML-like
operation on IA64).

A lot of the code in arch/*/mm/hugetlbpage.c is quite similar. This patch
attempts to consolidate a lot of the code across the arch's, putting the
combined version in mm/hugetlb.c. There are a couple of uglyish hacks in
order to covert all the hugepage archs, but the result is a very large
reduction in the total amount of code. It also means things like hugepage
lazy allocation could be implemented in one place, instead of six.
Tested, at least a little, on ppc64, i386 and x86_64.
Notes:
- this patch changes the meaning of set_huge_pte() to be more
analagous to set_pte()
- does SH4 need s special huge_ptep_get_and_clear()??

ia64 and sparc64 hurriedly had to introduce their own variants of
pgd_addr_end, to leapfrog over the holes in their virtual address spaces which
the final clear_page_range suddenly presented when converted from pgd_index to
pgd_addr_end. But now that free_pgtables respects the vma list, those holes
are never presented, and the arch variants can go.

ia64 and ppc64 had hugetlb_free_pgtables functions which were no longer being
called, and it wasn't obvious what to do about them.
The ppc64 case turns out to be easy: the associated tables are noted elsewhere
and freed later, safe to either skip its hugetlb areas or go through the
motions of freeing nothing. Since ia64 does need a special case, restore to
ppc64 the special case of skipping them.
The ia64 hugetlb case has been broken since pgd_addr_end went in, though it
probably appeared to work okay if you just had one such area; in fact it's
been broken much longer if you consider a long munmap spanning from another
region into the hugetlb region.
In the ia64 hugetlb region, more virtual address bits are available than in
the other regions, yet the page tables are structured the same way: the page
at the bottom is larger. Here we need to scale down each addr before passing
it to the standard free_pgd_range. Was about to write a hugely_scaled_down
macro, but found htlbpage_to_page already exists for just this purpose. Fixed
off-by-one in ia64 is_hugepage_only_range.
Uninline free_pgd_range to make it available to ia64. Make sure the
vma-gathering loop in free_pgtables cannot join a hugepage_only_range to any
other (safe to join huges? probably but don't bother).

Initial git repository build. I'm not bothering with the full history,
even though we have it. We can create a separate "historical" git
archive of that later if we want to, and in the meantime it's about
3.2GB when imported into git - space that would just make the early
git days unnecessarily complicated, when we don't have a lot of good
infrastructure for it.
Let it rip!
