Current HAL start/stop bt_sco session only for incall mode -
rewrite logic to start/stop bt session when system switch to
one of the bluetooth devices.
Move bt_sco_active and pcm_sco_rx/tx to main audio_device structure.
Remove bt_sco_capture profile.

This is part of a cleanup series.
In the future, we should reconsider the struct usage and (maybe?) move
the members into one common place.

This just allocates the structre and creates the RIL connection. This is
per audio_device structure to make sure we to not run into issues with
audioserver threading.
